Please note, for patients under 18 years of age, a legal guardian must be physically present for the first visit to sign consent forms. Babysitters and caretakers are not legal guardians. For subsequent visits, an adult needs to present, but not necessarily a legal guardian. Adult patients with legal guardians must also have those legal guardians physically present and proof of legal guardianship must be established before a Heal house call may begin. If a legal guardian is unable to be physically present at the house call, the legal guardian may provide Heal with prior, written authorization that identifies by name an individual who has the power and authority to consent to the treatment of the minor patient. If the doctor arrives and a legal guardian isn’t present, the doctor will be unable to proceed with the house call and $99 will be charged to the credit card associated with the account used to book the house call.
